He is survived by: his wife Suzanne; his children, Stephen and Megan; his daughter-in-law Kyra; his grandsons, Jackson and Blake; his sister Dr. Kathleen Koljack; his sister-in-law Suzanne Smith; his nieces and nephews, John Smith (Amy), Lindsay Smith (Brooke), Kevin Koljack (Olivia), Katie Koljack, Claire Koljack and Kimberly Slagle (Corey); his great-nieces and great-nephews, Emma, Sophie Slagle, Ainsley, Marin Smith, Chase Smith (Bekah), Nicholas Smith, Connor Slagle and Kevin Michael Koljack, Jr. (KJ); his great-uncle Daniel Lee Medders; and also, Gabriel, Hunter Smith and Lilly Smith. He is also survived by many cousins, god-children, a posse of simulcast friends, colleagues and classmates.
In lieu of flowers, the family requests that donations be made in Michael's honor to St. Patrick's Episcopal Church.
