He is survived by: his former wife Judy Pease; his sons, Thomas Wayne Forrest Jr. (Tommy) and Michael Wayne Forrest; his wife Lillie Forrest; his son Gary Wayne (Jocelyn) of Owasso; his sister Louise Forrest of Barnsdall; his daughter-in-law Tina Kelley (Michael) of Barnsdall; his grandchildren, Amber Martin (Lance) of Leawood, Tori Forrest of Tempe, Andrea Pickering (Michael) of Wichita, Brittany Arnett (Austin) of Mid, Alexandria Mudd of Tulsa, Brianna Forrest of Moore, Jordyn Stephens (Dylan) of Skiatook, Tailor Deckard (Connor) of Skiatook, Samantha Kelley of Barnsdall, Cody Forrest (Arm) of Boston and Tyler Forrest (Chelsea) of Yuma; and his great grandchildren, Layla, Aria, Vada, Braelynn, Roman, Israel, Ollivander, Zepher, Hunter, Marley, Ryan and Callahan. He is also survived by nieces, nephews, cousins, and friends.
