He was predeceased by: his parents, Mark Smith and Barbara Maxwell (Gardner). He is survived by: his wife Melinda; his children, Melissa Smith, Shelby Smith (Emily), Daniel Hutchins (Britni) and Jason Faulkner; his grandchildren, Brody, Austin, Hayden, Hazel, Hudson, Jason, Katie, Pandora, Jaden and Brycen; his great grandchild Zarah; his siblings, Carolyn Dulaney and John Richard Clark; and his uncle Ernest Smith. He is also survived by numerous cousins and extended family.
