He was predeceased by: his sister Josefina Vazquez, MD. He is survived by: his siblings, Amie Simpson and Tulio Vazquez of Chicago, Illinois; his children, Elsa Davis and Hiram M Vazquez, MD (Sarah); his grandchildren, Tristan (Jessica Bailey), Lacey Warren and Lauren (Austin Pennington); and his great grandchildren, Evangeline, Emory, Taytum, Jackson, Sienna, Zen and Luca.
