He was predeceased by: his parents, Forrest Sullivan and Wilma Sullivan; his wife Sandra Sullivan; his daughter Melinda Janoe; his granddaughters, Mandy Janoe and Mazy Sullivan; and his sister Nancy Polk.

He is survived by: his children, Pamela Self of Antlers, Loretta, Mike Motley of Avinger, TX, Gale Sullivan of San Francisco, CA and Justin Sullivan of Greenwood, LA; and his grandchildren, Nancy Johnson Starlin of Atoka, OK, Mark Janoe, Mylaine Self, Melody Maxwell, Tanner Hallows, Cory Motley and Jake Sullivan. He is also survived by 11 great-grandchildren.
