He was predeceased by: his sister Jamie Floyd; his grandparents, AJ Floyd, Curtis and Aleene Martin; and his uncles, Martin, Tony Dicks and Erin White.
He is survived by: his parents, Andrew Jackson Floyd III and Glenda Fay Floyd (Martin); his children, Brock Floyd, Brier Floyd, Bo Floyd and Brooklyn Floyd of Adair, Oklahoma; and his siblings, Andrew Floyd IV and Jodie Floyd of Adair, Oklahoma. He is also survived by several nieces, nephews and cousins; and many other relatives and friends.
In lieu of flowers, the family asks that donations be made to the Curtis Floyd Memorial Fund at RCB Bank for his 4 children he left behind.
