He was predeceased by: his parents, John Mulichak and Mary Zayac Mulichak; his son Steve E. Mulichak; his siblings, Nancy Zelina Williams and Robert Mulichak; his siblings-in-law, Al Williams, Gail Clayton and Elaine Mulichak; and his grandparents, John and Nancy Mulichak.
He is survived by: his son Jack; his wife Carol Clayton; his sons, John "JD" Mulichak (Jeannie Herdman) of Boardman and Thom Mulichak (Wendy) of Huntington Beach, CA; his grandchildren, Jack Mulichak (Thea Hischak) of Columbus, Jessie Mulichak Costas (James LaSalandra) of Boardman and Jamie Mulichak Heidrich (Mitchell) of Fort Mitchell, KY; his great grandchildren, Gabbi, Gia Costas of Boardman, Xavier, Rowan Mulichak of Columbus and Quinn Heidrich of Fort Mitchell, KY; his niece Sally Kloes of Boardman; and his brother Eugene Mulichak (Sharon).
The Mulichak family has requested contributions be given to the Steve E. Mulichak Youngstown State Memorial Scholarship Fund in memory of their son.
