She was predeceased by: her father James Galida. She is survived by: her mother Judith Gowing Galida; her sons, Jase and James Carkido; her former husband Michael Carkido; and her sisters, Sharon Galida, Karen Allen, Patricia Petro and Stephanie Jones. She is also survived by numerous nieces and nephews.

In lieu of flowers, the family kindly requests that memorial donations be made to Amy's sons.
