He was predeceased by: his parents, Cortis Amyx and Jewell Amyx (Perry). He is survived by: his wife Michelle; his siblings, Danny Amyx (Karen) and Brenda Lewis (Ron); his nephew Nathan Lewis (Donna); and his nieces, Erin Baldwin (Turk), April Pollock (Derek) and Erika DeBorde (Jaime). He is also survived by extended family; and numerous friends.
In lieu of flowers, memorial contributions may be made in memory to Hospice of Dayton.
