He is survived by: his mother Jean Berkhouse (Weckesser) of Apple Creek; his daughter Kayla; his granddaughter Brooklyn; his brothers, Mike Berkhouse (Heidi) of Apple Creek and Steve Berkhouse (Julie) of Franklin, IN; and his nieces and nephews, Shea and Ben Berkhouse.
