He was predeceased by: his parents, Michael Hlivko and Mary Buchovecky Hlivko; his wife Jane Flaherty Hlivko; and his siblings, John, Pete, Mike, Joe, Charlie, Stephen, Alec Hlivko, Josephine Forquer, Mary Duray and Martha Damba.
He is survived by: his children, Kimberly Levite (Bernard) and Richard Hlivko (Lisa); his grandchildren, Jenee Levite, Benjamin and Joshua Hlivko; and his sister-in-law Florence Hlivko of Johnstown, PA. He is also survived by many nieces and nephews.
