He was predeceased by: his parents, Jerry, Sr. Cook and Helen Beato Cook; and his pet Luka Belle. He is survived by: his wife Anna Brake Cook; his daughters, Julie Constantine (Joseph) of Wintersville and Jamie Lyn Moore (Jacob) of Carrollton; his brothers, Jeffrey Cook (Betsy) of Wellsville, OH and Joseph Cook (Linda); his grandson Gunner Constantine; and his parents-in-law, Ermine and Rose Brake.
