She was predeceased by: her parents, Carl Chuba and Jean Chuba (Kowalchik); and her grandparents, John, Tressa Chuba, Andrew and Helen Kowalchik.
She is survived by: her brothers, Daniel (Elizabeth), Craig (Lisa Lee) and John (Carrie Marie); her nieces and nephews, Brianna, Andrew, Tyler (Zoey), Noah, Gina (William), Dominic, Nolan and Aulton; her great-nieces, Rowan and Regan; her aunts, Evelyn Jamison, Grace Chuba and Betsy; and her uncle David Bukovsky.
