She was predeceased by: her husband Michael L.; her parents, Anton Polk and Antonia Polk; her stepmother Jenny Krajc Polk; her brothers, Frank Polk and John Polk; and her stepsister Dorothy Cipriani.
She is survived by: her children, Anita Visoky (Kenneth), Gerard (Ayumi Kudo), Michael (Patricia), Catherine (Michael Klein), Teresa (Greg Gerth) and Vincent (Susan); her grandchildren, Jack Visoky, Victoria Sabetta, Cheryl, Christopher Melfi, Michael, Elizabeth Melfi, Sarah Melfi-Klein, Andrew and Eleanor Melfi; and her stepsister Eileen Armstrong.
