He was predeceased by: his father William Polk; his siblings, William Polk, Robert Polk and Bonnie Woessner (Polk); and his children, Amy Polk and David Polk.
He is survived by: his children, Gretchen Furlong (Polk), Scott McCoy, Sherri Jones (McCoy) and Megan Dennard (Polk); his grandchildren, Drew, Seamus, Daric, Deidra, Dakota and Delaney; and his great grandchildren, Charleigh, Magdalena, Maximus and Emerson.
