He was predeceased by: his parents, Peter Cozmyk and Osa Thelma Cozmyk (Nesbit); his son Kelley Cozmyk; and his sister Rita Hunt. He is survived by: his daughter Amy Hempfling; and his brothers, Roger Cozmyk, Michael Cozmyk, Jon Cozmyk and Alan Cozmyk. He is also survived by many grandchildren and great grandchildren.
