He was predeceased by: his parents, Elmer Andrew Cook and Martha Broomfield Cook; and his former wife Kristine Lentz Cook. He is survived by: his wife Julia E. Breech Cook; his son Andrew "Andy" Ray Cook (Angie); his stepson James Norman (Summer); his daughter Jenny Elaine Yazell (Bill); his stepdaughter Deeana Norman; and his sister Elsie Clark. He is also survived by fourteen grandchildren and fifteen great grandchildren.
