He was predeceased by: his father Jack Lusk; his son Matthew David Lusk; and his brother Fred "Butch" Lusk. He is survived by: his children, Alan Lusk (Kim Clark), Mark Lusk (Connie) and Lynn Levelius (Gary); his brother James Lusk (Jeannie); his grandchildren, Beau Lusk and Emma Lusk; his friend Nathan Hampton; and his caregivers, Jennifer Loudermilk, Rita Ford and Micaela Schilling. He is also survived by several nieces and nephews.
