He is survived by: his spouse Noreta Mae "Rita" Forsythe (Wilson); his children, Monford A. Forsythe III (Debra), Janine Francis, Gloria Craig (Rick Pleiman) and John W. Forsythe (Rhonda); and his sister Martha Doak (Daniel) of Florida.
