He was predeceased by: his parents, Oral Forman and Geraldine Forman (Barger); and his brother David Forman. He is survived by: his children, Belinda Barkley (Michael Fahning) and Joshua Forman (Kelli Huesman); and his siblings, Danny Forman, Johnny Forman, William Forman, Carol Forman and Gerri Ann Kolb. He is also survived by several nieces and nephews, and by many close friends.
