He was predeceased by: his mother Valerie J. Ruffner Hawk; and his grandparents, Charles, Emma Hawk, Max and Vivian Ruffner.
He is survived by: his father Jerry L. Hawk, Sr. of Allensville; his siblings, Jarrod Hawk (Stephanie) of Wellston and Misty Hawk of Chillicothe; his uncles, Ray Hawk of Allensville and Rick Ruffner (Jeanette) of Logan; and his nieces and nephews, Brittany McNichols (Nick), Jaylah Kennedy, Ryan Hawk (Brittany), Jacob Hawk, Jordan Hawk, Braylon Hawk and Jace McKenzie. He is also survived by 3 great-nieces, 1 great-nephew, and several cousins.
