He was predeceased by: his parents, Frank E. Woltz, Jr. and Jean Marie Lawrence Woltz. He is survived by: his sister Lois Clark (Doug) of Wapakoneta, Ohio; his niece Holly Phillips (Todd) of Wapakoneta; and his nephew Douglas Clark, Jr. of Richmond, Virginia. He is also survived by great-niece; three great-nephews; and six cousins.

Memorial contributions may be directed to the Garnet A. Wilson Public Library of Pike County.
