He was predeceased by: his parents, Anthony Cook and Sara Cook; his wife Beverly (Schmitz); and his son Matthew H. Cook. He is survived by: his children, Deborah Dowling (Greg), Christine Ann Cook, John R. Cook, Robert M. Cook M. D. and Andrew P. (Susan); his grandchildren, Adam Cook, Anthony Cook (Jennifer) and Lauren Huizenga (Jordan); his great grandchildren, Jackson J. Cook and Charlotte M. Huizenga; and his sisters-in-law, Patricia Schmitz and Mary Rohr. He is also survived by numerous nieces and nephews.
In lieu of flowers donations can be made to Shriners Hospital for Children or St. Jude Children's Research Hospital.
