He was predeceased by: his parents, Samuel Ernest and Celia Evelyn Cook (Thomas); and his son-in-law Jesse Chitwood, Sr.. He is survived by: his wife Sharon Cook of Warren, OH; his children, Julie Cook, Sinkuc, Michael Samuel Cook (Kristen), Jonathan Thomas Cook, Sara Caspary, Megan Danko, Joseph Danko, Xyari Williams and Jamie Rolland (Craig); his sister Doris Lynn Davies (Wayne); his grandchildren, Jesse (Ali), Haley, Rylee, Jace, Jiovanni, Jaida, Juliet, Trenton, Kami, Jaxtyn, Blaize, Jozie, Brantley and Nash; and his aunt Mary Callahan.
Visitation was held on Tuesday, April 15th 2025 from 4:00 PM to 6:00 PM at the Carl W. Hall Funeral Home (533 N Park Ave, Warren, OH 44481). A funeral service was held on Tuesday, April 15th 2025 at 6:00 PM at the same location.
