She was predeceased by: her parents, George Axtell and Gladys Axtell (Peterson); her husband Howard J. Hawks Sr.; her siblings, Sarah Junk, Marilyn Tustin, Shirley Mae Axtell, Donna Young, Margaret Arch, Kathryn Longwell and George Axtell, Jr.; and her granddaughter Chasity Campbell.
She is survived by: her daughters, Shirley Stark (Shawn) of Niles, Evelyn Milleson (Randy) of Howland and Sarah Hnida (Bill) of Pulaski, PA; and her siblings, Grace Battles of Greenville, PA and Arthur Axtell (Bea) of Greenville, PA. She is also survived by 23 grandchildren; 42 great-grandchildren and 7 great-great-grandchildren.
In lieu of flowers, the family asks that donations be made to Hospice of the Valley in Phyllis's memory.
