He was predeceased by: his parents, Eugene Folk and Mariam Folk (Slusser). He is survived by: his daughters, Caitlin Holtzapple (Brock) of Medina, OH, Lorien Walker (Dan) and Wapakoneta; his grandchildren, Kelsie, Gavin Holtzapple and Keaton Walker; and his sister Shirley Hollis (L. V.).
