He was predeceased by: his parents, John Fayak and Erma Fayak; and his sister Elizabeth Betty Ann. He is survived by: his wife Susan; his daughter Yvonne Kitko (Greg) of Wakeman; his grandsons, Joshua (Akia) and Matthew (Emily); and his siblings, John "Butch" Fayak (Marlene), Robert Fayak (Roseann) and Carol Thompson (Bill).
