He was predeceased by: his parents, Herman Allen and Laura Mae Church; and his siblings, Robert Church, Helen Fuquay and Ette Heerdink.
He is survived by: his children, Robert Church of Toledo, Ohio, Keith Church (Kathy) of London, Ohio, Paul Church (Pam) of Birmingham, Ohio, Sara Bentley (John) and Lori Dillon (David) of Wakeman, Ohio; and his sister Dorthy Martin of Evansville. He is also survived by thirteen grandchildren; twenty-one great grandchildren.
