He was predeceased by: his father Leo Smith; his son Scott Smith; his sister Jody Smith; his nieces, Sarah Slone and Elise Ulmer; and his brother-in-law Rick Poetter.

He is survived by: his mother Bonnie Smith (Miller) of Wadsworth; his wife Margo Smith of Wadsworth; his sons, Matthew Smith (Cindy) of Barberton, Dan Smith of Nashville, TN and Jordan Smith (Carina) of Wadsworth; his daughter-in-law Megan Clifford Smith of Wadsworth; his grandchildren, Jackson, Maxton, Alexia, Lily, Olivia, Leo, Sophia and Cooper; and his siblings, Jim of Wadsworth, Steve of Wadsworth, Tom Smith of Wadsworth, Mary Ellen Poetter of Wadsworth, Sue Fajt (Jim) of Wadsworth and Barb Lesure (Al) of Wadsworth. He is also survived by numerous nieces, nephews, and friends.

In lieu of flowers, memorial contributions may be made to the Pulmonary Fibrosis Foundation, 230 E. Ohio St. Suite 500, Chicago, IL 60611.
