He was predeceased by: his parents, Victor Perkins and Vera Perkins; his brother-in-law Lawrence; and his grandson Jeremy. He is survived by: his friends, Don and Leroy; his wife Dorothy; his children, Stan (Judy), Duane (Cindy) and Pamela Shelley (Mark); his grandchildren, Chad (Nona), Henry, David and Lydia; his great grandchildren, Hattie Jo and Jude; his siblings, Richard (Glenna), Joyce Carter and James Michael (Nancy); and his uncle Frank. He is also survived by many nieces, nephews, and cousins.

In lieu of flowers, donations may be made to Summa Hospice in memory of Dave.
