He was predeceased by: his father Ron Hawk. He is survived by: his mother Patti Hawk; his wife Nicole (Kerns); his daughter Leise; his brother Rob; his nieces and nephews, Courtney and Tyler (Ivy); his uncles, Mike and Bob Durham; and his parents-in-law, Gwen and Mike Kerns. He is also survived by his extended family & many good friends.
