He was predeceased by: his parents, Glenn Melvin McIntosh and Alveria McIntosh (Humbarger); his wife Jeannette K. Rice; and his son Curtis L. McIntosh. He is survived by: his children, Victoria "Vicki" Sexton of Van Wert, Jeffery McIntosh of Montpelier and John McIntosh of Ohio City; and his grandchildren, Krystal Miller, Tyler McIntosh, Rachel McIntosh, Kinzie Steube, Alexandria McIntosh, Brooke McIntosh, Jeffrey Miller, Richelle Miller and Tonya Miller. He is also survived by 2 great granddaughters and 3 great grandsons.
