She was predeceased by: her parents, Leslie Brown, Sr. and Roxie Brown (Brown); her brother Dwayne Brown; and her grandmother Cora Blanche Phillips. She is survived by: her husband John Wallace of Van Wert; her daughters, Emily Wallace (William Levicky) of Powell and Jill Welch (Lucas) of Decatur, Indiana; her grandchildren, Hayley Schlatman, Olivia Welch, Logan Welch and Grace Levicky; and her siblings, Leslie Brown, Jr. and Jenny Nofer (Mark).
In lieu of flowers, memorial contributions in Cindy's memory may be made payable to the family (Emily Wallace) to be forwarded to Nationwide Children's Hospital Neonatal Intesive Care Unit.
