He was predeceased by: his parents, Wendell Hayslip and Martha Jane Hayslip (Herr). He is survived by: his wife Deborah Hayslip; his children, Erika Berry (Ryan) and Michael H. Hayslip; and his grandchildren, Oliver, Brody, Zachary, Zoe, Hailey, Kale and Kaitlyn. He is also survived by four nieces and nephews.
