He is survived by: his wife Jo Hazelton; his sons, Randy Hazelton (Leeann) and Brad Hazelton (Leigh); his sister Rhonda Caudill (Glenn); his grandchildren, Taylor Hazelton, Gabe Hazelton, Jeff Hazelton and Heather Hazelton; and his great grandchildren, Maykayla Hazelton, Zachary Hazelton and Wesly Hazelton. He is also survived by several nieces and nephews.
