He was predeceased by: his parents, Adran D. Smith and Marilyn Jean Smith (Beckett); and his brother Edward Smith. He is survived by: his wife Jacqueline L. Cavanaugh; his son Frederick Smith (Ronda); his stepdaughters, Kimberly Cannon, Julie Erickson and Kelli Giumenti; his grandchildren, Emma, Landon, Ty, Chase, Austin, Andrew, Steven and Cole; his sister Maurita Menapace; and his sister-in-law Lois Smith. He is also survived by many nieces and nephews.

In lieu of flowers, charitable contributions honoring his memory may be made to Community Hospice and Starlight Enterprises.
