He was predeceased by: his parents, Paul Blevins and Freida Blevins (Brown); his son Brack Kimble Blevins; his siblings-in-law, Morice and Margaret Witt; and his close friends, Dallas Retherford and Harley Vice.
He is survived by: his wife Janet Blevins (Witt); his daughter Cindy Pelfrey (Bobby); his grandchildren, Brack Blevins, Holly Jones (Steven) and Bobby Pelfrey (Micah); his great grandchildren, Stevie Jones, Scarlett Jones and Maci Lynn Pelfrey; his sister Donna Moore (Tom); and his pets, Zoey, Bowie and Lexie.
Memorial Donations may be made to the American Heart Association in memory of Sam.
