He was predeceased by: his parents, Leonard Patrick and Anne Derksen (Schooler); and his brother Thomas Patrick. He is survived by: his wife Janis L. Patrick (Chamberlain); his daughters, Brandi Ballantyne (Rob) and Annie Patrick (Collin Hammonds); his grandchildren, Katie Ballantyne, Connor Ballantyne and Parker Ballantyne; his brother Tim Patrick (Brenda); his nieces, Sarah Martin (Steve) and Dani Slagle; and his nephew Michael Patrick (Crystal).
