He was predeceased by: his parent Lucille White; and his cousin Ernie White. He is survived by: his son Shea Cook; his girlfriend Kathy Creekmore; his stepchildren, Amy L. Creekmore (Ray Stacy), John M. Creekmore III and Jenny Smith (Aaron); his grandsons, Bennett and Corbin; his brother Elvis Laminack; and his cousins, Rochelle White, Gina White, Jamie Burgy (Tony), Brian White, Jr. (Nicole), Marilyn Maidlow (Leo) and Butch Freeman (Donna). He is also survived by many other family members and dear friends.
