He was predeceased by: his wife Shirley Sullivan Polcyn. He is survived by: his children, Michael Polcyn (Janis) and Patricia Rose (Timothy); his grandchildren, Stephanie Schmidt, Jennifer Thompson (Adam) and Andrew Polcyn; and his great grandchildren, Evelyn Schmidt and Sapphire Thompson.

Memorial donations may be given to Christian Home Care for their caregiver fund in gratitude for their dedicated and compassionate service to Dad.
