He was predeceased by: his brothers, Martin Kubiak and Robert Kubiak. He is survived by: his wife Phyllis Kubiak (Izbinski); his children, Mary Jo Trojnacki (Peter) and Thomas Kubiak (Peggy); his grandson Ian Kubiak; and his brother Leo Kubiak (Sue). He is also survived by nieces and nephews.
