He is survived by: his parents, Gerald Ray and Carolyn Lee Phillips (Wilson); his brother Steven M.; his sister-in-law Mickie Phillips of Tipp City; his nephew Eric (Lauren Phillips); his great-nephew Owen; his great-niece Olivia; his niece Emily (Andrew Moss); and his great-nephews, Alex and Everett.

If so desired, memorial contributions may be made to St. Judes Children's Hospital.
