He was predeceased by: his wife Joan Baird (Ulrey). He is survived by: his sister Gene Anne Potter of Edmund, WA; his sons, Jason Baird of Tipp City, OH and Jeremy (Cindy Baird); and his grandchildren, Cole, Carson, Chloe and Jackson.
In lieu of flowers the family has requested donation be made in loving memory of Jack to either Hospice of Miami County or the American Cancer Society.
