She was predeceased by: her parents, Norville F. Elkins and Freda P. Elkins (Hudson); her husband Samuel J. Van Horn; and her siblings, Clarence P. Elkins, Max Elkins, Donald L. Elkins, Mary Lou Hooper and Wanda Hetich.
She is survived by: her sons, Mark A. Van Horn (Jill) of Macedonia, Scott B. Van Horn (Jeannie) of Newton Falls, Matthew S. Van Horn (Sara) of Tiffin and Paul D. Van Horn (Sharon) of Tiffin; her sister Geraldine L. Cass of Heath; her grandchildren, Jaclyn, Brittany, Rachel, Luke and Mark; and her great grandchildren, Jeramiah, Sierra, Ruby, Eliza, Noelle, Sloane, Ellis, Claire, Emmitt and Kenna.
