He is survived by: his parents, Joshua Ortiz of Maryland and Jennifer K. Logan of Tiffin; his daughter Brayleigh Shook; his siblings, Josiah, Julian Ortiz, Javoni Delph, Katelin Logan, Lorin Ross, Jazmin (Ciara Ortiz) and Jessica Kern (Neajha Kimble-Ross); and his grandmothers, Nola Logan and Martha Ortiz (Frank).
In lieu of flowers, memorial contributions may be made to his daughter, Brayleigh Shook in c/o the funeral home.
