He was predeceased by: his grandparents, Clair, Patsy Cook, Robert and Roberta Davis. He is survived by: his parents, Krista Coffman (Eric) of Fostoria and Trent Cook (Renee) of Risingsun; his girlfriend Abbe Norville; his sons, Wyatt, Bo and Waylon Cook; his siblings, Carter Coffman, Brent Williams (Tiffany), Carley Coffman and Skylar Cook; his grandparents, Tim, Kathy Beckman, Cindy Coffman and Junior Coffman; his niece Ember Williams; and his nephew Bryson Cook.
Memorial contributions can be made to the Cook family in care of Old Fort Bank for Cody's boys.
