He was predeceased by: his parents, George Timothy and Julie A. Bazar (Gaietto); his grandparents, Anthony Sr. Gaietto (Rosina) and George Bazar (Mary); his uncle Anthony Gaietto Jr.; and his aunt Laurie Henige (Gaietto).

He is survived by: his brother Douglas M. Bazar (Heather) of Romulus, MI; his aunts, Angie Brock of Tiffin, Nancy Johnson (Ed) of Longwood, FL, Jeannie Gaietto of Tiffin and Delores "Dee" Bazar of St. Clairsville, OH; his uncle Daniel Henige of Mansfield; and his close friend Marianna Hardy of Chagrin Falls.

In lieu of flowers memorial contributions may be made to the St. Mary's Catholic Church or to St. Meinrad Arch Abbey.
