He was predeceased by: his wife Lou Anna; his parent Rev. Clarkson Mayhew (Gladys); and his siblings, Mart Mayhew (Teresa), Clark Mayhew (Quata), George Mayhew, Ruth Conard (Kenneth) and Inabel Walvoord (Roger).
He is survived by: his children, Alan Mayhew (Linda Young) of Fort Worth, TX, Paula Rogerson (Greg) of Copley, OH and Linda Hallinan (Kevin) of Dayton, OH; his grandchildren, Shannon, Colleen Hallinan, Lauren, Megan and Steven Rogerson; and his sister-in-law Marilyn Mayhew.
