It is with great sadness that we announce the death of Chad Michael Haley of Sylvania, Ohio, who passed away on October 8, 2021, at the age of 51, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He was predeceased by: his father Elwood Haley (Woody). He is survived by: his mother Sharon Haley (Don); his wife Kristie Haley (Orth); his sons, Logan and Tyler (Logan); and his brothers, Allen Haley (Maggie), Brian, Darin (Stacey) and Eric (Laci).
