He was predeceased by: his parents, John Fondale and Theresa Fondale; and his sibling Dale. He is survived by: his wife Janice; his siblings, Dominic (Sandra), Linda (Tom), Carol (Kyle), Cindy and Debbie (Mark); his sisters-in-law, Beverly and Pam; his grandchildren, Tatiana, Victoria, Bianca, Rocco and Demi; and his sons, David Fondale, Kevin Fondale (Jodi), Jason Fondale (Heather) and Brett Fondale.

In lieu of flowers, donations can be made to the Wounded Warriors Foundation.

“Dave, we have been friends since we were little kids, lots of memories. We were in the Army and Vietnam like most of the other Patriots of our generation. We lived through a whole lot of History and now it's time to say see ya later. I'm sure you are riding on a cloud up there looking down on all of us.
